- Page 1 should be completed and signed by the parent and student athlete.
- Page 2 should be completed by a physician.
- Page 3 should be read by the parent and student athlete.
- Page 4 should be read and signed by the parent and student athlete.
- In order for a physical to be valid during the 2009-2010 school year it must be completed on or after May 1, 2009.
